Please be aware that in order to receive food, you will be required to fill out an application upon arrival. We will need such as names of all household members and dates of birth for each person in your household will be required on the application. (applications in Spanish available upon request)
Once you fill out your application, we will give you a number based on how many people are in your household. This determines how many boxes of food you will get each Monday.
After your application is entered into the system, you can go to any food bank registered through the United Food bank program one time per day (as long as it's open) to get food.
You will be required to show a current, unexpired government -issued photo like a drivers license, state ID, passport, or military ID each week.

Other options are to download an application here, fill it out, save it to your computer, and print it. Be sure to bring it with you when you come to the foodbank and turn it in when they go to check you in. This will make the check in process faster on your first visit.
*Please note: If you have more people in your household then there are spaces, please use the excess white space for the remaining members names and Dates of birth.
